We work it out before writing code
We talk to the people who will use the system, look at what already exists and find the places where things usually break. You get a work list and dates, not generalities.

We split the system into parts that can change independently, and think through what happens when there are ten times more users.

The project has a lead, tests are written alongside the code, and every release goes through checks. Once a week you get a report with numbers in it.
